- Download the application to your computer. You will need Adobe Reader to view the application. You can download Adobe Reader from the
Adobe website. - Sign application. Unsigned applications will not be accepted.
- Submit payment with application.
- Submit two digital photos or sketches of the device via email or CD.
- Submit application in one of the following ways:

- If completing application by hand, do so legibly and in blue ink. Illegible applications may delay processing times.
- When submitting an application electronically: You will be prompted to select an email client. Choose the option that is best suited to you (Desktop Email Application or Internet Email). Follow the directions on the screen. Once email is set-up, the application will attach to an email. Select "Send" and the application will be sent via email to NCWM. This may take a minute, so please be patient. If you do not receive an email confirmation from NCWM within five business days, your application most likely was not received by NCWM and you will need to resend.

Non-Technical Editorial Change
This application should be used if you would like to change the information listed in the "Submitted By" box on the Certificate of Conformance (CC) or to correct for errors contained on the CC, such as emin or nmax. This application does not extend to adding new information such as adding new models. All applications are subject to individual consideration.

Private Label
This application should be used if a company is marketing a device from a manufacturer and re-labeling it under its own name. A separate application is required for each National Type Evaluation Program Certificate of Conformance.

Reactivation
This application should be used if you would like to reactivate an inactive NTEP Certificate of Conformance. The reactivation must be made within 12 months of the date the Certificate became inactive. No testing is required.

Sale of a Company
This application should be used if a company has purchased the assets of a manufacturer who's weighing or measuring device is covered by a NTEP Certificate of Conformance and wishes to transfer the original manufacturer's Certificate of Conformance to its name. A separate application is required for each NTEP Certificate of Conformance.

Grain Analyzer Applications
Grain Analyzer Phase I
This application should be used if you do not currently have an active grain analyzer NTEP Certificate of Conformance (CC) for a specific model; you wish to add additional feature(s) to an existing active CC such as adding test weight per bushel; or you wish to add an additional or updated near infrared calibration to an existing active CC.

Grain Analyzer Phase II
This application should be used if you currently have an active Grain Analyzer Certificate of Conformance (CC) for moisture or moisture and test weight per bushel. Phase II testing is performed annually to update the calibrations of devices that are successfully evaluated in Phase I. You must participate annually in Phase II to maintain a current NTEP Certificate of Conformance. All requests for Phase II testing must be submitted by May 15.

Liquid Measuring Device
This application should be used if the device or equipment is designed to measure and deliver liquid. Taximeter
This application should be used if the device automatically calculates at a predetermined rate or rates and indicates the charge for hire of a vehicle. This application does not apply to odometers on vehicles that are rented on a distance basis.

US/Canada Mutual Recognition Agreement for Retail Motor-Fuel Dispenser
This application should be used if applying for a NTEP Certificate of Conformance (CC) under the National Type Evaluation Program and a Notice of Approval (NOA) under section 3 of the Canadian Weights and Measures Act for devices covered by the US/Canada Mutual Recognition Agreement on Type Evaluation. Each country continues to issue its own CC and NOA. Acceptable devices: gasoline and diesel dispenser, high-speed dispenser and gasoline and diesel meters intended to be used in fuel dispensers and truck refuellers only.

Automatic Bulk Weighing System
This application should be used if a weighing system is adapted to the automatic weighing of bulk commodities in successive drafts of predetermined amounts automatically recording the no-load and loaded weight values and accumulating the net weight of each draft (i.e. a hopper batching system).

Belt-Conveyor Scale
This application should be used if a device that employs a weighing element in contact with a belt to sense the weight of the material being conveyed and the speed (travel) of the material, and integrates these values to produce total delivered weight.
This application does not apply to devices used for: discrete weighing while moving on conveyors, measure quantity on a time basis, check weighers, controllers or other auxiliary devices except as they may affect the weighing performance of the belt-conveyor scale.

US/Canada Mutual Recognition Agreement on Type Evaluation Scale
This application should be used if the device is 1) electronic computing and non-computing bench and floor scales with a capacity up to 1 000 kg (2 000 lb) 2) separate weighing/load receiving elements with a capacity up to 1 000 kg (2 000 lb) 3) separate electronic indicators (except those that are "software based," i.e., programmed by downloading parameters) or 4) mechanical scales up to 10 000 kg (20 000 lb).
